Cross Reference Table does not match an existing single row

Have a Cross Reference Table with a singe row. The map that has the CRT look up function, cannot locate and find the single row. Why?
1. Ask customer if the cross reference table was imported from a CSV file or manually inserted the rows.
2. Most likely the rows were inserted manually.
3. Copy the map, and the cross reference table from customer account to your account, and open up the XML representation.
4. Most likely some of the referenced columns are missing.
